Forms

Below you will find general Youth Ministries registration forms. We ask that each student/family register at the beginning of the school year so that we can keep our database information current. Simply print, fill out and turn in.

FUMC Medical Release Form is required for all students who are participating in off site events. It is good for a calendar year.

MIddle School Registration Form is required for all students who are 6th - 8th grade. It is good for the current school year.

High School Registration Form is required for all students who are 9th - 12th grade. It is good for the current school year.

Parent Interest Form is requested of each family. It helps us better utilize family volunteers.

Confirmation Forms

 The confirmation classes are now full.

Medical Release Form This is the same form as above. You only need to fill out one per calendar year.

Retreat Form This is for our two retreats. If returned to the youth office in a timely manner you only have to fill it out once.

• Students: CTCYM form (2 sided)  FUMC Medical Release form (2 sided)
• Adults/Young Adults: CYCYM form (2 sided)
